Discussion: Building a Health History
Health assessment is an essential part of treatment since it allows clinicians to gather patients’ health-related information needed during diagnosis and treatment. Healthcare providers build patients’ health histories through communication and interview techniques. This discussion focuses on communication and interview techniques used in building a health history for a 22yo LGBTQIA female Hispanic who is an immigrant and resides in a middle-class suburb. Additionally, the discussion includes how interview questions will be targeted based on social determinants of health, appropriate risk assessment instruments, potential health-related risks, and five targeted questions for the selected patient.
A patient’s health history collected during health assessment significantly depends on communication and interview techniques utilized. According to McCorry and Mason, (2020) healthcare providers should select communication strategies based on an individual patient to enhance understanding and establish a good physician-patient relationship. In collecting health history from a building a 22yo LGBTQIA female Hispanic the healthcare professional would utilize written strategies. The client is from a different diverse ethnic and linguistic background. Therefore, using written strategies will enhance communication between the provider and the patient, leading to a better provider-client relationship. Additionally, the healthcare provider should utilize verbal communication. This communication technology will enable the clinician to ask the client various questions, collecting the required patient’s health-related information. Lastly, the healthcare provider should utilize attentive listening skills. The patient will be encouraged to provide the required health-related data if the clinician listens attentively to the client without interrupting.
The questions for building a patient’s health history will be based on several factors, including the client’s social determinants of health, age, gender, and ethnicity. First, the question will be asked if she has experienced any disease outbreak in the suburbs where she resides. Secondly, the patient will be asked if her social status and income level limit her from accessing quality healthcare services. Furthermore, the client should be asked if she has a history of substance use disorder diagnoses or mental disorders. Additionally, risk assessment instruments should be used in assessing patient’s potential health risks. The most appropriate risk assessment instrument for this client is Drug Abuse Screening Test (DAST-10). According to Hughto et al. (2021), the prevalence of drug and alcohol abuse in the United States is disproportionately high among LGBTs. Therefore, being an LGBT, Drug Abuse Screening Test (DAST-10) is the most appropriate risk assessment instrument to reduce the risk of substance use disorder diagnoses (SUDDs).
Lastly, the healthcare provider should utilize the following targeted questions in assessing patient’s health risk.
Have you ever been diagnosed with addiction, alcohol use disorder, or substance use disorder diagnoses?
Have you or your family members been diagnosed with chronic illnesses such as diabetes, hypertension, or any cardiovascular disease?
Is there a likelihood of contracting the infectious disease in your residence?
Have you ever been diagnosed with sexually-transmitted infections? If yes, were you treated?
Do you practice a healthy lifestyle, including getting adequate sleep, taking a balanced diet, and engaging in physical activities?
